Chronic Condition Care at Home in Montgomery County, MD

A diagnosis such as Alzheimer’s disease, Parkinson’s disease, stroke, COPD, or diabetes can change much more than a person’s medical needs. It can affect mobility, memory, independence, daily routines, and a family’s ability to safely manage care at home.

Advanced Nursing & Home Care provides personalized in-home support for individuals living with chronic, neurological, and memory-related conditions throughout Montgomery County, Maryland. Our care is built around the individual — their diagnosis, abilities, routines, preferences, and the level of support they need to continue living safely and comfortably at home.

Whether your family needs help managing everyday activities, skilled nursing oversight, support following a hospitalization, or more comprehensive ongoing care, our team can develop a care plan that adapts as needs change.

Specialized Care For Complex Conditions

If medication timing, fall risk, or keeping up with day-to-day changes is starting to feel like more than you can manage alone, we’re happy to talk it through — no pressure, just a conversation about what support could look like right now.

Common Reasons Families Call Us

Most families don’t begin looking for home care because of a diagnosis alone. They call because something has changed.

A parent with dementia has started wandering. Parkinson’s is making it harder to move safely around the house. A loved one is coming home after a stroke and the family isn’t sure how they will manage the recovery. A chronic condition is becoming more difficult to monitor. Or the person who has been providing most of the care realizes they can’t continue doing everything on their own.

Families often contact Advanced Nursing & Home Care when:

  • A loved one’s condition is making everyday activities more difficult.
  • Changes in memory or behavior are creating new safety concerns.
  • Mobility, balance, or fall risk has become a concern.
  • Medications or medical needs are becoming more complicated.
  • A loved one is returning home after a hospital or rehabilitation stay.
  • A chronic condition requires more consistent monitoring and support.
  • A family caregiver needs additional help to safely manage care at home.
  • A condition has progressed and the current care arrangement is no longer enough.

You don’t have to determine the right level of care on your own. We can talk through what is happening, help you understand your options, and develop a plan based on your loved one’s current needs — with the flexibility to adjust that care as those needs change.

Frequently Asked Questions

We support people living with dementia and Alzheimer’s disease, Parkinson’s and other neurological conditions, recovery after a stroke, COPD, diabetes, heart failure, and other chronic conditions. Care is built around how the condition affects the person at home, not just the diagnosis.

Often, yes, with the right support in place. Many people living with dementia, Parkinson’s, or other chronic conditions stay home safely with help for daily routines, supervision, fall prevention, and medication reminders, with skilled nursing added as needs become more complex. We start with an in-home assessment to understand what’s needed.

Yes. Our team has access to Parkinson’s-specific education through our Parkinson’s Foundation Community Partner program, and many of our care providers complete dementia care training. We match each client with a care provider whose experience fits their condition and needs.

Yes. Care can start with a few hours a week and grow to include overnight care, 24-hour care, or skilled nursing as a condition progresses. The care plan is reassessed as needs change, so support can be increased, reduced, or adjusted without starting over with a different provider.

Our care supports the treatment plan your loved one’s physician sets; it doesn’t replace it. Care providers help with daily routines and report changes. Licensed nurses (RNs and LPNs) carry out ordered clinical tasks like medication management and chronic condition monitoring. Diagnosis and treatment decisions stay with the physician.
