How We Match You With the Right Nurse

Every skilled nursing client goes through a thorough intake process before care begins. We take the time to understand the medical needs, daily routines, and personality of the person receiving care — then match them with an RN or LPN whose skills and experience fit that specific situation. Our nurses go through careful screening before joining our team, so every match is built on both clinical competence and genuine fit.

Coming Home Shouldn’t Mean Losing Clinical Support

If your loved one is being discharged with new medications, a wound care plan, IV therapy, or ongoing monitoring for a chronic condition, we can have a licensed nurse in place quickly — often before you’ve had time to fully process the hospital discharge instructions yourself. Whether it’s managing a new medication schedule, giving injections, tending a healing wound, or keeping an eye on a chronic condition, our nurses pick up exactly where the hospital left off, so nothing falls through the cracks during the transition home.

Not Sure If You Need Skilled Nursing or Personal Care?

Many families aren’t sure whether their loved one needs hands-on medical care or help with daily routines; often, it’s both, at different times. That is exactly what we help families figure out.

Personal Care — Helps with day-to-day living: bathing, dressing, meal prep, light housekeeping, companionship, and mobility support. Provided by trained, compassionate care providers — no nursing license required, because the need isn’t medical.

Skilled Nursing Care — Involves hands-on clinical tasks: medication management, wound care, IV therapy, injections, and monitoring chronic conditions. Provided only by licensed RNs or LPNs, because the need is medical.

Many families start with one and move to the other as needs change — recovering from surgery might mean skilled nursing at first, then personal care once healing is further along. Or the reverse: personal care for years, with skilled nursing added only when clinical tasks become part of the routine. We can help you figure out exactly where your loved one is right now.

What Happens After You Contact Us?

Call, and you’ll speak directly with a member of our care team — not a call center — who listens to what’s happening and what kind of help you’re looking for.

We assess your loved one’s medical needs, daily routine, and home safety. That’s what determines the right level of care — personal care, skilled nursing, or both.

Based on that plan, we match your loved one with a care provider or a licensed nurse suited to their specific needs and personality — never assigned at random.

Your care provider or nurse reviews the full care plan before their first visit, so nothing gets explained for the first time at your door.

Within 2–4 days of the first visit, our Client Satisfaction team checks in to confirm the care is the right fit and nothing has changed — then keeps checking in as needs do.

If your loved one’s needs change—a new diagnosis, a hospital stay, more hands-on help—your Staffing Coordinator adjusts the care plan rather than starting over.

Frequently Asked Questions

Skilled nursing care is medical treatment and monitoring provided at home by licensed registered nurses (RNs) and licensed practical nurses (LPNs). It includes medication management, IV therapy, wound care, ventilator and tracheostomy management, and transitional care, with every client matched to a nurse suited to their specific medical needs.

Skilled nursing involves hands-on clinical tasks, such as medication management, wound care, IV therapy, injections, and monitoring chronic conditions, and is provided only by licensed RNs or LPNs. Personal care helps with daily living, such as bathing, dressing, meals, and mobility, and doesn’t require a nursing license. Many families use both at different times or at the same time.

Only on a limited, short-term basis. Medicare-covered home health requires a physician’s order, homebound status, and care from a Medicare-certified agency, and it doesn’t cover ongoing nursing support. We don’t bill Medicare or Medicaid directly. We provide private duty skilled nursing care for as long as it’s needed, typically paid privately, through long-term care insurance, or through VA benefits.

how families pay for home care >

Yes. Our skilled nursing team supports clients with complex, higher-acuity medical needs, including ALS, MS, and other neurological conditions, ventilator management, and tracheostomy care, under RN supervision.

We coordinate with another qualified RN or LPN to step in, so care continues without interruption. Your loved one’s care plan stays the same, and the covering nurse is briefed on it. We can walk you through how continuity works for your specific plan when you call.
